    Responsible for coordination of MDS completion with other involved departments. Responsible for completing the MDS Nursing Areas as well as any other designated areas of MDS. Responsible for scheduling PPS assessment dates and completion of PPS assessment, with other involved departments. Responsible for locking of MDS/PPS and submissions. Provides supervisory and operational support of the nursing home as directed by the DON.
    Provides education and evaluation to nursing and resident care staff.

    Licensure/Certification/Registration: Currently licensed as a Registered Nurse in the State of Minnesota

    Education: Minimum A.D. Nursing Diploma with current licensure in the State of Minnesota

    Experience: LTC experience preferred. Experience with MDS/Care planning process. Ability to assess health status. Computer usage helpful. Experience in supervision.

Facility Wide

  • Infection Prevention/Outreach Services Coordinator: 1 Full Time

    OUTPATIENT MANAGER:
    Demonstrate an overall understanding and commitment to the facility’s mission and values. The Outreach Manager is responsible for the overall day to day operation and management of patient services in the Outpatient Area, including but not limited to: Telemedicine, Specialty Clinics, Outpatient IV Infusions, Outpatient Procedures, Outpatient Services, and Marketing.

    Qualifications:
    Licensure/Certification/Registration: Basic Life Support and Advanced Cardiac Life Support
    Education: Registered Nursing License in the State of MN (Bachelor’s preferred)
    Experience: IV Insertion and medication delivery experience, experienced and proficient working with IT equipment and computer programs, including Word, Excel, Power Point, and Microsoft Office. Management experience preferred. Organization skills are required. Excellent Customer Service Skills. Minimum of 2 years of clinical nursing experience preferred.

    INFECTION PREVENTION:
    To prevent exposure and contact of infectious disease within the Cook Hospital & Care Center and consumers by providing quality patient care, limiting morbidity and mortality from healthcare acquired infections, providing patient, employee and visitor education and providing a safe healthcare environment that promotes and supports quality patient care.

    This expectation is achieved through systematic data and surveillance, preventive methodology and control strategies that reduce the risk of transmission and contact of infectious disease as well as promotes an optimally safe environment for the patient and the health care worker.

    This position acts as an authority to identify infection prevention issues and has the ability to implement corrective actions.

    Qualifications:
    Licensure/Certification/Registration:
    • Must be licensed as a Registered Nurse (RN) in the State of Minnesota
    • Current Infection Control Certification (CIC) or CIC within three years of hire if novice Infection Prevention Practitioner
    • Current certification in the “Nursing Home Infection Preventionist Training Course” or within 90 days of hire

  • Screening Team Member: 1 Part Time

    This is a direct employee/patient-service position as a front-line screening team member. 

    Primary responsibilities include screening employees/patients/visitors for temperature, possible COVID-19 symptoms

    and previous exposure to a COVID19 confirmed case. The screening team member will also be responsible for

    disbursement of surgical and/or handmade masks to all entering the facility. 

     

Central Supply / Staffing Coordinator-Scheduler

  • Central Supply Assistant/Staffing Coordinator: 1 Full Time

    The Central Service Assistant assumes delegated responsibility and accountability for all aspects of duties pertaining to the Central Service Department.

    The Staffing Coordinator/Scheduler is responsible for scheduling nursing personnel to work in the Care Center and Hospital.

    Qualifications: High School graduate, prefer secondary education in clerical and/or business. Prefer previous nursing/healthcare training and experience. Prefer materials mgmt./purchasing training or related experience.
    Experience in scheduling of staff preferred.
