Environmental Services

  • Housekeeping & Laundry: Casual

    The environmental Tech I is responsible for housekeeping and laundry duties within the facility.

  • Housekeeper: 1 Full Time & 1 Part Time

    Position Summary:  Responsible for the overall cleanliness of the facility. To maintain a safe, clean, infection free environment throughout the facility.

    Qualifications:

    Licensure/Certification/Registration:  No license required

    Education:  High School Diploma or equivalent

    Experience: Experience in healthcare housekeeping  preferred

Imaging

  • X-ray, CT, & EKG: Full Time & Casual

    Performs X-rays; CTs and EKGs.  Experience preferred, but will train the right candidate.

    Graduate of an accredited program of Radiologic Technology; Registered with ARRT or State of MN X-ray Operators License issued prior to 2007.

     

  • Radiologic Technologist: Contract - weekend rotation

    Weekend only rotation

    Private call room with DISH TV, refrigerator and own bathroom.  Meals provided.

    Hours of coverage – Friday evening through Monday morning – Hours TBD

    Performs X-rays; CTs and EKGs.  Experience preferred, but will train the right candidate.

    Graduate of an accredited program of Radiologic Technology; Registered with ARRT or State of MN X-ray Operators License issued prior to 2007.

Dietary

  • Dietary Aide: 1 Casual

    The Dietary Aide would be responsible for washing dishes, general cleaning, tray line positions and meal distribution.

  • Dietary Aide/Cook: 1 Full Time & 1 Part Time

    The Dietary Aide would be responsible for washing dishes, general cleaning, tray line positions and meal distribution.

    The cook would be responsible for cooking meals.

Business Office/Admissions

  • Secretary / Receptionist: Casual

    The Interdepartmental Secretary provides secretarial services to facility departments. This may include all activities included in a general office position, including but not limited to:  keyboarding, filing, photocopying, telephone coverage, collating in-service documents, etc.

    Licensure/Certification/Registration: None required. However, must be or become proficient with the Meditech Admissions module. Notary Public commission desirable.

    Education:   High school diploma, higher education or experience in a related field preferred. Must be skilled in general office procedures, type a minimum of 50 WPM, have excellent spelling, punctuation and grammar skills. Knowledge of Microsoft Office programs required.

    Experience:   One year’s experience with general office practices and working with the public in some capacity is desirable.

     

     

     

Activities

  • Activities Assistant: 2 Part Time

    Assist the Activity Director in managing and directing the activities of the activity department to provide a physical, psychosocially stimulating environment through an appropriate on-going program of activities. The activities are directly related to the residents and swing bed residents needs, wants, and interests of the Cook Hospital and Care Center.

Care Center

  • Nursing Assistant: Part Time

    The Nursing Assistant is a caregiver who assists professional nursing personnel in providing resident care and maintaining efficient function for a designated time frame.

  • Restorative Nurse Aide: 3 Casual

    To complete restorative nursing programs assigned by therapists. Responsible for the implementation of the Restorative Rehab Program for the facility.  Serves as the liaison between formalized therapy and nursing.  Promotes a restorative nursing philosophy among all departments.

Maintenance

  • Maintenance / Plant Operations: 1 Full Time

    Position Summary: Responsible for maintaining a safe and clean environment throughout the facility. Also, maintaining the equipment, tools, furniture, etc. of the facility. Follow all tasks as set forth by the Plant Manager.

    Licensure/Certification/Registration: Possess, at a minimum, specialist boiler license and must become second class C certified within 6 months of hire.

    Education: High school diploma or equivalent.  

    Experience: Knowledge of boilers, general plumbing, electrical, construction, johnson controls and HVAC systems preferred.

     

Rehabilitation

  • Occupational Therapist: Full Time

Our Physical Therapists provide the assessment, treatment, planning, implementation, education, communication and leadership to meet the physical therapy needs of our patients.

    The Cook Hospital Physical Therapy Department has been meeting the therapy needs of our community for the past 40 years. The Physical Therapy Department has a combined 55 years of experience working across the life span from pediatrics to geriatrics with a diverse patient population to include but not limited to:
    In patient and out-patient Orthopedics
    Sports Medicine
    Neurologically Disorders
    Industrial Medicine

    The Occupational Therapist is directly responsible to the Director and/or the Occupational Therapy Supervisor or designee for non-direct patient care issues. The Occupational Therapist has the responsibility for overall direction, utilization, supervision, instruction, and evaluation of professional and supportive staff, volunteers, and students in their area of the occupational therapy department/facility.

    Licensure/Certification/Registration: Minnesota Department of Health Licensure; verification of passing score through National Board for Certification of Occupational Therapy (NBCOT). Continued certification is optional; NBCOT certification must be maintained for use of Registered in OTR title.
    Education: Bachelor’s degree in Occupational Therapy minimum.
    Experience: Minimum of two years experience preferred.
    Must have verification card or proof of current instruction in CPR.
    Must meet continuing education requirements for Occupational Therapy profession.
