Care Center

  • Certified Nursing Assistant: Part Time

    The Nursing Assistant is a caregiver who assists professional nursing personnel in providing resident care and maintaining efficient function for a designated time frame.

  • RN/LPN: Part Time

    The staff Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for patients for a designated time-frame and provides care to these patients via therapeutic use of self, the nursing process, the environment/instrumentation and other health care team members.

    The Licensed Practical Nurse (LPN) is a caregiver who assumes delegated responsibility and accountability for patient/residents for a designated time frame and provides care to those patients/residents via therapeutic use of self, the nursing process, the environment, instrumentation and other health care team members.

  • MDS Coordinator: Part Time

    MDS stands for Minimum Data Set. MDS coordinators maintain records of patients and make sure that the documents adhere to both federal and state regulations. They also work closely with administrative and medical staff so as to keep patient records up-to-date and mostly handle the resident assessment instrument or RAI process for the patients from their admission up to discharge.

  • PIPP Nurse: FTE .4

    The PIPP (Performance-based incentive payment project)

    Registered Nurse (RN), Licensed Practical Nurse (LPN) is a professional caregiver who assumes responsibility and accountability for implementing the facility designed PIPP program in the Care Center. The PIPP nurse is to implement, evaluate, educate and assess the program to ensure goals as indicated in the PIPP proposal. The PIPP nurse will evaluate to ensure goals are being met, help track progress and submit reports as needed to the MDH. The PIPP nurse will participate in QOL meetings. Initiate psychosocial program as well as new PIPP projects.

Environmental Services

  • Housekeeping & Laundry: Casual

    The environmental Tech I is responsible for housekeeping and laundry duties within the facility.

Rehabilitation

  • Physical Therapist: Full Time

    We have a full time opportunity as a Staff Physical Therapist – graduate from a physical therapy program accredited by the American Physical Therapy Association, licensed or eligible for license to practice Physical Therapy in Minnesota. Provides the assessment, treatment, planning, implementation, education, communication and leadership to meet the physical therapy needs of patients. Fosters working relationships among professionals and ancillary staff. The physical therapist may serve clients of all ages. Skills in Orthopedics, Neurological and Pediatrics treatment preferred.

Imaging

  • X-ray, CT, & EKG: Casual

    Performs X-rays; CTs and EKGs.  Experience preferred, but will train the right candidate.

    Graduate of an accredited program of Radiologic Technology; Registered with ARRT or State of MN X-ray Operators License issued prior to 2007.

     

  • Radiologic Technologist: Contract - weekend rotation

    Weekend only rotation

    Private call room with DISH TV, refrigerator and own bathroom.  Meals provided.

    Hours of coverage – Friday evening through Monday morning – Hours TBD

    Performs X-rays; CTs and EKGs.  Experience preferred, but will train the right candidate.

    Graduate of an accredited program of Radiologic Technology; Registered with ARRT or State of MN X-ray Operators License issued prior to 2007.

Dietary

  • Dietary Aide/Cook: 1 Part Time

    The Dietary Aide would be responsible for washing dishes, general cleaning, tray line positions and meal distribution.

    Under limited supervision, the cook assumes responsibility for preparing high-quality foods according to standardized recipes. Complies with all State and Federal guidelines.

  • Dietary Aide: 1 Casual

    The Dietary Aide would be responsible for washing dishes, general cleaning, tray line positions and meal distribution.

Adult Day Services

  • Adult Day Service Director: Casual

    Responsible for total direction and management of the Adult Day Services Program including compliance with pertinent regulations as they apply to ADS. Develop an ongoing process of patient recruitment as well as keeping attuned to possible funding resources.  Promote a positive public image consistent with overall mission and philosophy of the organization.

    Licensure/Certification/Registration: LPN

    Education: Current LPN licensure in the State of Minnesota

    Experience: Nursing experience working with adult clients required. Previous experience in planning and/or delivering health and social services to adult clients preferred.  Supervisory experience in health care preferred.

     

Business Office/Admissions

  • Secretary / Receptionist: Casual

    The Interdepartmental Secretary provides secretarial services to facility departments. This may include all activities included in a general office position, including but not limited to:  keyboarding, filing, photocopying, telephone coverage, collating in-service documents, etc.

    Licensure/Certification/Registration: None required. However, must be or become proficient with the Meditech Admissions module. Notary Public commission desirable.

    Education:   High school diploma, higher education or experience in a related field preferred. Must be skilled in general office procedures, type a minimum of 50 WPM, have excellent spelling, punctuation and grammar skills. Knowledge of Microsoft Office programs required.

    Experience:   One year’s experience with general office practices and working with the public in some capacity is desirable.
