RN Case Management
The Cook Hospital and Nursing Home have an RN Case Manager in the hospital as well as the nursing home. The Case Manager assists with admissions, referrals, discharge planning, as well as ongoing management of the nursing home residents. The Case Manager is a part of the interdisciplinary team that meets daily to promote quality care for our patients and residents.
