SOCIAL SERVICES The Cook Hospital and Nursing Home has a Social Services Department that serves both the hospital and the nursing home. A Social Worker assists with admissions, referrals and discharge planning, as well as ongoing case management of nursing home residents. The Social Worker is part of the interdisciplinary team that meets daily to promote quality care for the nursing home residents and hospital patients.
