Care Center

  • Certified Nursing Assistant: 3 Part Time

    The Nursing Assistant is a caregiver who assists professional nursing personnel in providing resident care and maintaining efficient function for a designated time frame.

Hospital & ER

  • Registered Nurse: 2 Part Time/12 hour shifts

    The staff Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for patients for a designated time-frame and provides care to these patients via therapeutic use of self, the nursing process, the environment/instrumentation and other health care team members.

  • Unit Clerk/Nursing Assistant (HUC): 1 casual

    The Cook Hospital is currently seeking a candidate to work in our Hospital as a Casual Unit Clerk/Certified Nursing Assistant. Responsible for coordinating efficient unit function, reception, secretarial and patient care.

    Licensure/Certification/Registration: Active and in good standing with the MN
    Nursing Assistant Registry. Current BLS Certification.
    Education: High School Diploma or equivalent; Ward Secretary, Unit Assistant or Medical Records
    training required.
    Experience: Prior Hospital experience preferred. Prior secretary experience in a health care setting.

  • Cardiac Rehab Coordinator: 1 Casual

    Demonstrate an overall understanding and commitment to the facility’s mission and philosophy. The Cardiac Rehab Coordinator will provide patient services in the area of cardiac rehab and assist in the evaluation and treatment of patients 13 years and older who are referred to the Cardiac Rehabilitation Program Phase II at the Cook Hospital.  Will assist with the planning, implementation, evaluation, management, budgeting, and marketing of clinical operations of Cardiac Rehab.  Serves as a liaison for patients with physicians and referral sources.

    Licensure/Certification/Registration:  Basic Life Support & Advanced Cardiac Life Support

    Education: Bachelor’s Degree in exercise physiology or Registered Nursing license in the State of MN (Bachelor’s preferred)                Experience: Exercise program planning.

    Supervision and counseling with cardiac rehab patients.

    Knowledge and experience in EKG interpretation.

    Competent in adult education principles.

Adult Day Services

  • Adult Day Services Assistant: 1 Casual

    Assist the Adult Services Program Coordinator in managing and directing the activities of the Adult Day Services Department to provide a physical, psychosocially stimulating environment through an appropriate on-going program of activities and services. The activities are directly related to the participant needs, wants, and interests of the Cook Hospital.


  • Dietary Aide: 2 Casual

    The Dietary Aide is responsible for washing dishes, general cleaning, tray line positions and meal distribution.

  • Culinary Chef: 1 Full Time

    Prepares, directs and is responsible for meal service and special functions/holidays. Keeps kitchen, storage, and dining room clean. Keeps records as directed.


    • Graduate 2-4 year culinary program.
    • ServSafe certification
    • Good written and verbal communication.
    • Demonstrates effective communication skills with patients, families, visitors, peers, supervisors, and all other persons associated with Cook Hospital & Care Center.
    • At least one year experience in quantity cooking preferably in a similar setting.

Environmental Services

  • Housekeeping & Laundry: 2 casual

    The environmental Tech I is responsible for housekeeping and laundry duties within the facility.


  • Physical Therapist: 1 Full Time

    We have a full time opportunity as a Staff Physical Therapist – graduate from a physical therapy program accredited by the American Physical Therapy Association, licensed or eligible for license to practice Physical Therapy in Minnesota. Provides the assessment, treatment, planning, implementation, education, communication and leadership to meet the physical therapy needs of patients. Fosters working relationships among professionals and ancillary staff. The physical therapist may serve clients of all ages. Skills in Orthopedics, Neurological and Pediatrics treatment preferred.
