What Is DAYBREAK?
Daybreak is the name of the Adult Day Services program in Cook, Minnesota and is located in the Cook Hospital and C&NC Unit.
Daybreak is an interactive program designed especially for older adults to provide a place to visit, work, and have fun together.
What’s It All About?
The goal of the Adult Day Services program is to enable older adults to continue to enjoy quality lives, maintain independence, and provide a much-needed respite for families and caregivers. Interaction with another human beings is vital to remaining healthy for anyone, but it becomes especially important as we age.
Daybreak provides activities and programs that give participants the opportunity to develop confidence, self-esteem, self-expression, and to build on and maintain current living skills.
Daybreak’s goals are to support the participant’s highest level of functioning, as well as encourage well-being and dignity, all in a safe and stimulating homelike environment.
The Purpose
Through the program, participants socialize with their peers through a variety of interactive activities and receive assistance with daily needs — all in an atmosphere that is welcoming, nurturing and most of all, fun.

Fees
We accept private pay, financial assistance through long term care insurance, VA, community service contracts or Minnesota waivered programs. We will be happy to assist you in finding the best financial options available.
Transportation
Transportation options are available. We offer pick up and drop off service in the Cook area. Other options include community based services, local volunteers and family members. We will be happy to discuss your transportation options with you.
Hours of Operation
9:30 a.m. - 2:30 p.m. Monday - Friday, exclusive of holidays.
Services are available on a full-time or part-time basis.
