PATIENT SERVICES Cardiac Rehab | Patient Medical Records | Patient Financial Services CARDIAC REHAB PHASE II Cardiac Rehabilitation Phase II Program provides a wonderful environment for the heart patient to adjust and discover what it takes to resume a healthy and active life. Our program is designed for those who have myocardial infarction (Heart attack), coronary artery bypass graft (CABG) or other heart surgery, angina, PTCA (percutaneous transluminal coronary angioplasty)/Stent, heart or lung transplant, or other cardiac related issues. Cardiac Rehab provides heart monitoring, prescribed exercise program, diet counseling, stress reduction, and a variety of education sessions. The program can last up to 12 weeks.
